CONVOCATION 2012 : NAME TO BE PRINTED ON THE DEGREE SCROLL

Please be informed that the name to be printed on your degree scroll will be similar to your name as it appears on your Examination Results slip. In this regard, please check and ensure that the spelling of your name on your Examination Results slip is correct.

Should your name on your Examination Results slip differs from that as stated in your MyKad / Pasport, please complete the Request for Change of Name Form attached and submit it to the Examinations Section, University of Malaya together with a copy of your MyKad / Pasport latest by 30th August 2012.

If we do not receive any request from you on this matter by 30th August 2012, the name to be printed on your degree scroll shall follow that which is stated in your Examination Results slip. Subsequent request to amend your name after your degree scroll has been printed shall not be entertained.
